Annual ED visits account for a sizeable portion of primary care physician’s (PCP) services. In California, PCPs are required to meet minimum Clinical Practice Guidelines (CPG) services standards for exceptional visits in order to be eligible to receive reimbursement. The percentage of exceptional visits each year have been increasing over time. However, the characteristics of exceptional ED patients are unknown. We sought to identify what proportion of ED self-referrals received exceptional visits, and to compare patients receiving exceptional visits to non-exceptional patients. We also sought to assess whether a patient’s PCP distance from an ED affects their receipt of exceptional visits. Self-referrals were categorized as exceptional or non-exceptional based on whether their median distance was the first percentile or greater than the first percentile. We calculated the percentage of exceptional visits as a proportion of all visits to the ED to determine disparities. Using a random sample of 105,142 PCPs from the California Medical Association, we performed multivariable logistic regression analysis to identify patient, physician, and practice characteristics that were associated with exceptional visits, adjusting for age and gender. Among 142,636 unique ED self-referrals, 14,488 (10.2%) patients received an exceptional visit. Patients receiving exceptional visits were older, were more likely to have a prior catastrophic illness or injury, were less likely to have an immunization history, and were more likely to have had care from a PCP for three or more months and to be Medicaid beneficiaries. A small number of ED self-referrals received exceptional visits. Thus, efforts to encourage more PCPs to meet the CPG requirements for exceptional visits may improve access to primary care and avoid unnecessary ED visits, particularly among the elderly, the homeless, and the uninsured.The meteoric rise of the cryptocurrency market caused a strange phenomenon.
